Before starting your formal application, please ensure you have the following documents available.
Indonesian from Local Schools in Indonesia
1. Photograph
A colour photograph of the applicant (3x4 cm, white background, JPEG file)
2. Birth Certificate, Family Registration card and Passport
A copy of the applicant's Birth Certificate and Family Registration card A copy of passport (cover page, picture page, vital information page, expiration date - minimum of 18 months validity) of applicant and parents (PDF file)
3. Affidavit Letter and Study Permit from Ministry of Education & Culture (for Indonesian who holds dual citizenship)
A copy of Affidavit Letter and Study Permit letter from Ministry of Education & Culture (PDF file)
4. Last 2 years report from previous school
A copy of school record for the past 2 years where the applicant is old enough to possess them, in English or officially translated in English (PDF, in a ZIP file)
5. Reference Letter from previous school (only applicable for Year 3 and above applicants)
As we need to send out the “Academic Reference Request” and “Student Safeguarding and Child Protection Declaration Form” please provide your child’s previous school admissions email address. The school will need to fill in and sign, in order to proceed with your application process.
6. National certificate of completion - Ijazah nasional SD and/or SMP- only for students transferring to Secondary starting from Year 8
A copy of national certificate of completion (PDF file)
7. Transfer Letter from Ministry of Education & Culture ( for applicant from local schools outside Banten province)

8. Learning Support Records (for applicants with Individual needs/supports)
A copy of English language of Learning Support reports/evaluations (PDF file)
9. Immunisation Records
A copy of immunisation records which describes the type of and the date when the vaccination was taken, including the Covid-19 vaccination (PDF file)
Indonesian from Overseas Schools
1. Photograph

3. Recommendation Letter from Embassy and copy of Passport
A copy of Recommendation Letter from Embassy and copy of Passport (PDF file)
4. Last 2 years report from previous school
School record for the last 3 years where the applicant is old enough to possess them, in English or officially translated in English (PDF, in a ZIP file)
5. Reference Letter from previous school (only applicable for Year 3 and above applicants)
